    Two of the most challenging clinical situations in psychiatry are accurately detecting a history of hypomania and determining whether a depressed patient has unipolar or bipolar depression. This is crucial in determining treatment and avoiding worsening the patient's condition with the wrong medication.The fact that on average the correct diagnosis of Bipolar II takes 10 years of seeing psychiatrists attests to this difficulty . Chapter 3, gives the best guide to detection of hypomania history I have used. Chapter 4 is an excellent guide in differentiating bipolar depression from unipolar depression. Chapters 5,6, and 7 give an excellent summary of treatment of elevated mood. Chapters 8 through 14 deal with "special situations" . I recommend this book to all practicing psychiatrists.
